A northeast Nebraska man is accused of masterminding a massive medical fraud case for more than a million dollars in bogus claims.
Mark Koehler of Norfolk is the former business manager at a Norfolk health care provider, called Heartland Physical Therapy.
Federal prosecutors say Koehler commited bank fraud and health care fraud.
The U.S. Attorney’s office alleges he submitted $1.4 million in false claims to Medicare, Nebraska Medicaid and to private insurers.
Koehler’s also accused of defrauding a Norfolk bank by putting the fake billings in reports used for cash advances of a half-million dollars.
Koehler reportedly left the company nearly two years ago.
