The Occupational Safety and Health Administration assessed the fine after issuing a serious safety violation against Wanzek Construction, headquartered in West Fargo, North Dakota.
OSHA issued the violation after investigating the death of a 40-year-old crane operator May 27th.
Thomas Bales of Denver, Colorado didn’t have on safety restraints, according to the OSHA investigation, when operating a 90-ton rough terrain crane. The crane slipped into a ditch, overturned, and crushed Bales as it rolled off an embankment. Bales had been on the jobsite for only 10 days.
Wanzek was working on the Prairie Breeze Wind Energy Center for Invenery near Elgin when the fatal accident occurred.
